Chronic Condition Management During the Age of COVID

Many of us have seen the secondary impact of COVID-19 on our patients with chronic conditions. Some providers have come up with creative ways to ensure quality care for these patients. In this forum, providers from Regions 3 and 5 of Colorado discuss how they addressed gap closure, engaged patients from the periphery (attributed but unengaged), provided care coordination across systems (specifically primary care and behavioral health), and the use of innovation in providing primary care..

Watch the Substance Use Disorder (SUD) Provider Forum, created in collaboration with community partners.

  • SUD Opening: See opening remarks from the Colorado Department of Health Care Policy and Financing (HCPF) followed by a forum agenda, an overview of Medicaid by HCPF and the functions of behavioral health organizations.
  • MSO: See an overview of the Managed Service Organization (MSO) system; MSO clients; how providers can access services; what MSOs pay for; and contact information.

The Centers for Disease Control and Prevention published the first national study of lesbian, gay, bisexual and questioning youth and unhealthy behaviors. Learn more about the CDC’s Youth Risk Behavior Surveillance System (YRBSS), which monitors six categories of priority health-related behaviors that contribute to the leading causes of mortality and morbidity among youth and adults in the United States.

Watch the Health Care Interpreter Network’s (HCIN) training video entitled Qualified Interpreting for Quality Health Care: A Training Video for Clinical Staff on How to Work with Interpreters. This 19-minute film covers topics like why it’s important to use a qualified interpreter instead of “getting by”; cultural considerations; key protocols for language interpreting, including confidentiality and first-person interpreting; and tips for using remote interpreters.

Cultural Responsiveness

Cultural responsiveness is a component of diversity, equity and inclusion (DE&I). The cultural responsiveness training consists of six short videos on various DE&I components. The videos are not health care specific, but an introduction to specific topics to foster additional conversations with your team. Within a lunch hour, you will be able to complete all videos.
