Health and Other Support

Advance directives are written instructions that state your wishes about your health and medical care. They are used if you are not able to make health care decisions for yourself. Click here to learn more.

If you have Health First Colorado (Colorado’s Medicaid program):

If you have Health First Colorado, there are two ways you can change your PCP.

We are sorry, but we are not able to help you change your PCP if you have Health First Colorado.

If you are experiencing a mental, substance use, or emotional crisis, help is there when you need it.

Emergency care is for sudden, unexpected health issues that need immediate care. If you don’t get care, you could have a serious injury to your body’s functions or organs. Or you could put your health at serious risk. If you are pregnant, the health of your unborn baby could be at serious risk too. If you’re having an emergency, call 911. Or go to the nearest emergency room.

Supplemental Nutrition Assistance Program (SNAP): This program helps you purchase healthy food. They also have training on nutrition and food preparation.

The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC): This program offers free healthy food benefits. It also offers breastfeeding support and nutrition education. These are for those who are pregnant and breastfeeding and have children under age 5.

Schedule Transportation with Intelliride

If you have Health First Colorado, you can get rides to and from your health care appointments. You have options:

  • Mileage reimbursement
  • Public transportation
  • Private vehicle or taxi
  • Wheelchair or stretcher van
  • Other options may also be available

Transportation can only be used for non-emergent health care appointments. This means things like a doctor’s office, the hospital, or another medical office. Do not use it in an emergency.

If you need a new Health First Colorado ID card:

If you need a new CHP+ ID card:

  • Call our customer service team at 800-511-5010.

If you need to report your CHP+ ID card as stolen:

If you are having an emergency, call 911 or go to the emergency room. If you need medical help fast but it’s not an emergency, call your doctor first.

If you have Health First Colorado or Child Health Plan Plus (CHP+), make sure they have the right contact details for you. This means your address, phone number, and email address. This will help you get important updates about your health care coverage.

It’s easy to update your contact details. Here are the ways you can do this:

  1. Visit colorado.gov/PEAK. If you don’t have a PEAK account, you can make one there.
  2. Use the free Health First Colorado app on your phone. You can download it from the Apple App store or the Google Play store. Visit healthfirstcolorado.com/mobileapp to learn more about the app.
  3. Get help from our Access Medical Enrollment Services team. Visit accessenrollment.org to learn how they can help you. Or call them at 855-221-4138.
  4. Contact your county department of human services. Visit cdhs.colorado.gov/our-partners/counties/contact-your-county-human-services-department to find out how to contact them.
  5. If you have CHP+, call CHP+ customer service at 800-359-1991. Or use State Relay 711. They can help in multiple languages.

How do I find a primary care provider (PCP) or specialist who takes my insurance?

We’re happy to help you find a provider who accepts Health First Colorado (Medicaid) or Child Health Plan Plus (CHP+). You can search for a provider online using our provider search. You can also call us and we’ll help you find a primary care medical provider or a specialist. Please call us at 800-511-5010, Monday through Friday from 8:00 a.m. to 5:00 p.m.
