Enhanced Primary Care Case Management
Effective March 1, 2010, members of Colorado Access Health Plan—Enhanced Care Management will be transitioned to Colorado Access Enhanced Primary Care Case Management.
Colorado Access Enhanced Primary Care Case Management is a partnership between Colorado Access and State Medicaid. As a member of this plan, you get all of the State Medicaid benefits and personalized service from a Colorado Access care manager.
Benefits
As a member of Enhanced Primary Care Case Management, you are eligible to receive all State Medicaid benefits. For a full list of State Medicaid benefits, visit State Medicaid's Web site.

Who Do I Call?
If you have questions or need help and are not sure who to call, please read the following section.
Call State Medicaid If You:
- Have general questions.
- Get a bill from a provider for services you received on or after March 1, 2010.
- Have a complaint or grievance.
- Need help getting your medications.
