Colorado Access has had a care management program in place for several years to help members with disabilities and chronic diseases improve their overall health. At the time the program was launched, health plan staff used an internally developed disease management tool to track outcomes for its highest-risk members. The program soon expanded to incorporate a utilization management platform to enable more robust case management. While the increased functionality was valuable, the use of multiple tools had become burdensome for users. "Many of our staff were spending time jumping back and forth between programs to enter or retrieve information," said Mike McKitterick, Senior Vice President of Clinical Operations for Colorado Access.
