How do I Become a Colorado Access Provider?

If you would like to become a Colorado Access provider, please e-mail the following information to Colorado Access Provider Contracting at pns@coaccess.com.

• Contact name, e-mail and phone number
• Address
• Provider specialty
• Provider Tax ID Number (TIN)

A representative from Provider Contracting will contact you after we review your information.
